Specialized Lymphedema Management & Post-Surgical Care

Lymphedema β€” the persistent swelling caused by damage to the lymphatic system during cancer surgery or radiation β€” affects thousands of Indian cancer survivors annually, significantly impacting quality of life. Our Lymphedema & Post-Cancer Rehabilitation service provides the gold-standard Complete Decongestive Therapy (CDT) programme, meticulously adapted for India's diverse climates, lifestyles, and cultural practices, delivered by internationally certified lymphedema therapists.

Complete Decongestive Therapy (CDT) Protocol

CDT is the internationally recognized gold-standard treatment for lymphedema, comprising four integrated components delivered across two phases by our certified therapists.

Phase 1

Intensive Decongestion Phase

Daily treatment sessions (typically 2–4 weeks) focusing on rapid volume reduction and limb softening, delivered in our clinic or through home visits across major Indian cities.

Phase 2

Maintenance & Self-Management Phase

Long-term self-care programme empowering patients to maintain their achieved outcomes through self-MLD, compression garments, skin care, and exercises β€” adapted for the Indian home environment.

The Four Components of CDT

Manual Lymphatic Drainage (MLD)

MLD is a precise, rhythmic, light-pressure massage technique that stimulates the lymphatic system to reroute excess lymph fluid away from congested areas through alternate lymphatic pathways. Our therapists are certified in both the Vodder and FΓΆldi MLD techniques. Each MLD session (45–60 minutes) begins at the neck's cervical lymph nodes and progresses systematically through the trunk and affected limb, following the anatomy of the lymphatic system.

Multi-Layer Compression Bandaging

Applied immediately after each MLD session during Phase 1, multi-layer short-stretch compression bandages maintain the volume reduction achieved by MLD and prevent refilling of the interstitial spaces with lymph fluid. The bandaging system comprises a skin-protective stockinette, foam padding layers, and short-stretch bandages applied from distal to proximal with carefully graded pressure gradients, as per established CDT protocols.

Lymphatic Decongestive Exercises

Specific, carefully prescribed exercises performed while wearing compression bandaging or garments create a muscle-pump effect that actively drives lymph fluid through the lymphatic vessels. Exercise programmes are tailored to the patient's cancer treatment status, age, fitness level, and functional goals β€” and are adapted for performance in Indian home settings without the need for gym equipment.

Meticulous Skin & Nail Care

Lymphedematous skin is highly susceptible to infection (cellulitis), which can rapidly worsen lymphedema and become life-threatening. Our therapy programme includes comprehensive skin care education: daily inspection, pH-balanced moisturising, nail hygiene, and immediate management of any breaks in the skin β€” all essential for preventing cellulitis in India's hot and humid climate conditions.

Lymphedema Management Tailored for Indian Patients

Standard international lymphedema guidelines often overlook the specific challenges faced by patients in India. We've developed India-specific adaptations that address your real daily-life realities.

Managing Bandaging in Hot, Humid Indian Climates

In coastal cities like Mumbai, Chennai, Kochi, and Kolkata β€” where heat and humidity regularly exceed 85% β€” standard compression bandages and garments can feel unbearable and worsen skin maceration, increasing infection risk. We address this with:

  • Lightweight, breathable compression textiles: Medical-grade, open-weave compression fabrics specifically engineered for tropical climates, balancing therapeutic pressure with breathability.
  • Strategic bandaging schedules: Wearing compression during cooler morning hours and activity periods, with rest and limb-elevation periods during peak afternoon heat.
  • Moisture-management underliner guidance: Moisture-wicking skin liners under compression bandages to reduce sweat accumulation during monsoon season.
  • Cooling protocols: Safe, evidence-based limb cooling strategies using water-dampened compression to reduce discomfort during extreme heat.

Modifying Activities for Indian Household Ergonomics

Indian daily life involves specific physical postures and activities rarely addressed in Western lymphedema guidelines. Our therapists provide personalised guidance on safely modifying:

  • Floor-level activities: Safely managing lymphedematous limbs while sitting cross-legged (sukhasana) for prayer, meals, and social activities.
  • Traditional kitchen tasks: Ergonomic modifications for pressure-cooking, grinding with a chakki, kneading atta dough, and lifting heavy vessels.
  • Saree draping and attire: Guidance on safely wearing a saree or salwar kameez with arm or leg lymphedema, including fabric choices and safe draping techniques.
  • Festival and puja activities: Managing limb protection during festivals involving prolonged standing, walking, carrying offerings, or floor-sitting.
  • Public transport in Indian cities: Protecting lymphedematous limbs in crowded buses, local trains (Mumbai local, Delhi Metro), and auto-rickshaws.

Dietary Guidance within Indian Food Culture

Diet plays a crucial role in lymphedema management. Our guidance is grounded in Indian food culture and avoids impractical Western dietary recommendations:

  • Low-sodium adaptations: Practical strategies for reducing salt in Indian cooking (reducing pickle, papad, and namkeen consumption) without sacrificing flavour.
  • Anti-inflammatory Indian ingredients: Incorporating therapeutic turmeric (haldi), ginger (adrak), and curcumin-rich foods that support lymphatic health.
  • Protein intake for wound healing: Practical, affordable, vegetarian and non-vegetarian protein sources (dals, paneer, eggs, fish) to support tissue healing post-surgery.

Compression Garment Access & Affordability in India

Medical-grade compression garments remain expensive and difficult to access across much of India. Our programme addresses this through:

  • Local sourcing network: Connections to verified Indian suppliers of OEKO-TEX-certified compression hosiery at accessible price points across major cities.
  • Ayushman Bharat & insurance guidance: Assistance navigating Ayushman Bharat PM-JAY coverage and private health insurance claims for compression garments and CDT therapy.
  • Garment care in Indian homes: Training on hand-washing compression garments with locally available mild detergents (Rin, Surf Excel Gentle) and drying methods suited to monsoon season.

Comprehensive Post-Surgical Rehabilitation

Guided recovery from the first post-operative day through to full functional independence.

Days 1–7: Immediate Post-Operative Care

Gentle positioning, breathing exercises, early active ankle pumping, prevention of deep vein thrombosis, and oedema monitoring β€” often initiated bedside in hospital wards at partner cancer hospitals.

Weeks 2–4: Wound Healing & Early Mobility

Progressive range-of-motion exercises, scar mobilisation (once healed), lymphedema risk education, and commencement of graded strengthening appropriate to the surgical site and oncological clearance.

Months 1–3: Functional Restoration

Targeted strengthening, functional task training (Indian ADL-specific), lymphedema maintenance programme (if applicable), fatigue management strategies, and safe return to work and social activities.

Months 3–12: Long-Term Survivorship

Annual surveillance reviews, exercise prescription for long-term health (addressing increased risks of cardiovascular disease and osteoporosis in cancer survivors), and psychosocial wellbeing support integrated with the physiotherapy journey.
